Employees may be deemed exempt from this requirement based on documentation from their health care provider.Job DescriptionHeading:Title: School Based CHWReports To: BH SupervisorDepartment: Behavior Health Job Grade: 8Employment Status: Non-ExemptDate: 6/14/2022General Responsibilities: The school-based community health worker is a member of the behavioral health team and will be embedded in a local middle school and high school to arrange telehealth behavioral health services to students during school hours. The CHW will closely collaborate with school guidance and student support staff to identify students in need of behavioral health services. The CHW will outreach families of identified students to provide support and care coordination needed to access behavioral health services. This position will require close collaboration with students, families, medical providers and Thundermist behavioral health staff. The CHW will be expected to reduce barriers to care, address basic needs and assess for social determinants of health.Required Qualifications: 1.Bachelor's degree preferred, High School diploma and case management experience will be considered. 2. CHW certified or eligible 3 Ability to work independently and effectively in a team environment4. Highly organized and strong interpersonal skills. 5. Maintain timely and accurate records and documentation as required.6. Work Schedule Demands: Full-time: school hours.
