Description
The Self-Direction Assistant (Qualified Intellectual Disabilities Professional) is responsible for providing case management to individuals with intellectual/developmental disabilities and families receiving the Medicaid Home-Based Waiver. This role involves arranging, directing, and managing services to promote independence for approximately 45-50 individuals. Collaborating with agency programs and external service providers, as well as connecting individuals and families with community resources, are essential aspects of this position. Upholding the Mission & Values of the Association is a key expectation.
Responsibilities:
* Meeting with Independent Service Coordination agencies for Discovery and/or Personal Plan
* Explaining all rights to individuals as required by DHS
* Adhering to timelines for completing Individual Support Plans, Person-Centered Plans, and assessments while ensuring accuracy of information in documents
* Recording monthly contacts with individuals, families, and community providers in approved agency format
* Completing necessary documentation and paperwork within specified deadlines
* Maintaining complete virtual files meeting regulatory expectations and confidentiality policies
* Providing practical skills training for individuals and families to manage Waiver services independently
* Facilitating connections to community resources and other providers when needed
* Managing waiver funding, monthly budget, maintaining Medicaid eligibility, and keeping records for compliance with IDHS
* Assisting individuals in obtaining adaptive equipment, assistive technology, home accessibility modifications, and vehicle accessibility modifications
Requirements
* Minimum of one year experience working directly with people with developmental disabilities
* Bachelor's degree in a Human Services field
Other Requirements:
* Excellent verbal and written communication skills
* Clear State Police History and Clear Health Care Worker Registry background checks
* Valid driver's license
* Minimum 21 years of age
* Successful completion of pre-employment drug screening
Schedule:
* Monday to Friday, 8:00 AM to 4:30 PM
