We are currently seeking a qualified Self-Direction Assistant (Qualified Intellectual Disabilities Professional) to join our team. As a Self-Direction Assistant, you will be responsible for providing case management to individuals with intellectual/developmental disabilities and families receiving the Medicaid Home-Based Waiver. Your role will involve arranging, directing, and managing services to promote independence for approximately 45-50 individuals. Collaborating with agency programs and external service providers, as well as connecting individuals and families with community resources, are essential aspects of this position. Upholding the Mission & Values of the Association is a key expectation.

Responsibilities:

* Meeting with Independent Service Coordination agencies for Discovery and/or Personal Plan

* Explaining all available services to individuals and families

* Developing and implementing individualized service plans

* Coordinating and monitoring services provided by external service providers

* Ensuring compliance with all applicable regulations and standards

* Maintaining accurate and up-to-date records

* Providing ongoing support and advocacy to individuals and families

* Collaborating with agency programs and external service providers to ensure the best possible outcomes for individuals and families

* Connecting individuals and families with community resources to promote independence and enhance quality of life

Requirements

* Bachelor's degree in a related field

* Certification as a Qualified Intellectual Disabilities Professional (QIDP)

* Experience working with individuals with intellectual/developmental disabilities and their families

* Knowledge of Medicaid Home-Based Waiver program and related regulations

* Strong communication and interpersonal skills

* Ability to work independently and as part of a team

* Strong organizational and time management skills

* Ability to maintain accurate and up-to-date records

* Commitment to upholding the Mission & Values of the Association

Schedule: Monday - Friday 7:30-4pm
