Essential Fiscal Intermediary Functions:
  • Meets with individuals and/or families interested in self-directed services, explains the role of Support Broker, Fiscal Intermediary representative and the agency. Secures necessary documentation based on referrals to engage with agency as their Fiscal Intermediary.
  • Establishes and maintains good working relationships and open communication with the self-direction participant's Care Coordinator (CC), Support Broker (SB), and Developmental Disabilities Regional Office (DDRO) SDS Liaison.
  • Abides by and ensures fluency of all OPWDD (Office for People with Developmental Disabilities), Self-Direction, Medicaid and associated programmatic regulations.
  • Reviews and submits all self-directed budgets ensuring required elements are in place prior to submission to the OPWDD.
  • Actively participate in Circle of Support Meetings (as needed) and Life Plan reviews as the FI representative.
  • Participate in any interviews of self-directed staff as needed. Assist broker and HR with hiring process.
  • Collaboration with the Support Broker that all training for self-hired staff is completed and monitored through their duration of employment.
  • Assist with the creation of applicable service plans and documentation as needed for both staff and the participant.
  • Monitors all documentation of services for compliance and provides all needed follow up and support with staff and participants to ensure compliance with documentation, billing and recording of payroll information.
  • Reviews and approves applicable employee timecards
  • Reviews and approves caseload billing
  • Reviews and approves all invoices/expenses as applicable related to approved expenses associated with and approved in each individual's self-directed budget.
  • Monitors monthly budgets of each individual in collaboration with the finance office.
  • Co-manages all self-directed/self-hired staffing (SDA) with the self-directed participant. The Fiscal Intermediary Representative will address any staff performance issues in accordance with agency policy, HR and the self-directed participant.
Essential Support Brokerage Functions:
  • Establishing and maintaining good working relationships and open communication with the participant's Care Coordinator (CC), Fiscal Intermediary (FI), and Developmental Disabilities Regional Office (DDRO) SDS Liaison.
  • Abides by all OPWDD (Office for People with Developmental Disabilities), Self-Direction Guidance, Medicaid and additional regulations.
  • Ensuring that there is a Support Broker Agreement in place that is consistent with the approved SDS Plan/Budget.
  • Assisting the participant in developing and maintaining a Circle of Support/Planning Team and assisting in planning and directing team meetings (this includes: assisting the participant in scheduling and facilitating team meetings; identifying new team members; identifying and assigning roles and tasks for Circle of Support/Planning Team members; assisting with the overall functioning of the Circle of Support/Planning Team; ensuring team members are freely chosen by the participant; and the team is operating in the participant's best interests and within the principles of Self-Determination).
  • Assist with the creation of applicable service plans and documentation as needed for both staff and the participant.
  • Ensuring completion of and updates to Staff Action Plans for self-hired Community Habilitation and Supported Employment (SEMP) staff for participants who receive such services.
  • Assisting the individual to develop a comprehensive Self-Directed Budget that is consistent with his/her Life Plan.
  • Attending the participant's Life Plan reviews and assisting the participant with reviewing and updating his/her Life Plan as requested.
  • Working with the individual and his/her Circle of Support/Planning Team to ensure that all necessary safeguards are included and addressed in the Life Plan.
  • Assisting the participant with developing a comprehensive Life Plan.
  • Ensures self-directed budget required elements are in place prior to submission to Fiscal Intermediary Representative (FI Rep) for review.
  • Providing education, training, and technical assistance to the participant and his/her Circle of Support/Planning Team in implementing the Self-Directed Budget according to applicable federal and state standards.
  • Working with the participant, his/her Circle of Support/Planning Team, and the Care Manager to identify and develop initial connections in the community, as identified in the participant's Life Plan.
  • Assist with monitoring Self-Direction expenditures to ensure that spending does not exceed the Self-Directed Budget by assisting the participant and the Circle of Support/Planning Team to review the expenditure report provided by the Fiscal Intermediary (FI).
  • Working with the individual and Circle of Support/Planning Team to review and update the Self-Direction Budget, as needed, so that it meets the needs of the participant and remains current and eligible for Medicaid funding.
  • Ensuring that at least two face-to-face Circle of Support/Planning Team meetings occur each year.
  • Assisting the participant with properly documenting services according to applicable federal and state regulations and policy (including assisting the participant with reviewing and submitting documentation to the FI, such as: employee time sheets; monthly summary note; mileage and expense reimbursement forms; and all other required documentation)
  • Assisting the participant with hiring and retaining appropriate support staff (including assisting the participant in recruiting, interviewing, hiring, scheduling, and supervising self- hired staff, and assisting the participant with identifying and retaining adequate backup staffing to ensure availability as needed by the participant).
  • Maintain Support Broker authorization on an annual basis, submitting recertification by the 31st of each July; submitting reauthorization approval to Director of Self-Directed Services.
Essential functions to both Fiscal Intermediary/Support Brokerage:
  • Maintains all records of self-directed participants' self-directed budgets, plans and other required documentation.
  • Performs any other duties as deemed necessary as requested
  • Conducts self in such a manner as to meet agency policies and standards defined in the Arc GLOW Staff Member Handbook at all times.
  • Ability to work in a variety of settings that include; office, homes of individual's served, and the community.
Non-Essential Functions:
  • Upholds and promotes the Mission, Vision, and Values of Arc GLOW.
  • Participates in corporate compliance and quality assurance
  • Participates in agency committees

Knowledge, Skills, & Abilities:
  • Demonstrate ethical business practices, in conformance with all state and federal laws and regulations.
  • Possess excellent verbal and written communication skills.
  • Previous experience working with diverse populations, including low to moderate-income families.
  • Ability to multi-task and prioritize duties.
  • Ability to travel in all weather conditions throughout the counties served.
  • Willingness to foster agency, department and program wide cooperation and team work through use of positive/constructive communication techniques.
  • Ability to maintain absolute confidentiality in regards to all records reviewed including client records, employee records and billing records.
  • Proficiency and experience with basic computer applications.
  • Ability to analyze and interpret data and to handle problem resolution.
Physical Requirements:Ability to meet the following physical requirements with or without reasonable accommodation:

Minimum Qualifications:
  • Bachelor's and/or Associate's Degree in Human Services or related field with one-year experience working with Individuals with Developmental Disabilities. High School Diploma with comparable education and/or experience may be considered.
  • Valid NYS driver's license with a safe driving record per agency policy
