The Senior Case Manager/Community Coordinator, under the supervision of the Program Director, plays a vital role in providing individual and group education while facilitating community integration for clients. This position involves participating in Support Planning to assess client strengths, needs, abilities, and goals, and educating clients about available services and training opportunities in the community. The Senior Case Manager/Community Coordinator is responsible for timely documentation in line with agency policies, coordinating and leading groups, and organizing special program events to foster community engagement and enhance social and emotional skills. Additionally, this role includes managing a caseload of clients, particularly high-risk individuals, based on clinical assessments and program needs. Essential Position Functions:
  • Collaborate with the Case Management team to identify individuals eligible or interested in vocational and educational services.
  • Manage a caseload that includes high-risk mental health and substance abuse clients, serving as a support resource for other Program Case Managers.
  • Provide coverage for community clients needing short-term advice or assistance.
  • Offer referrals and transportation assistance to clients for vocational, educational, and recreational needs, as well as support for the case management team.
  • Participate in service planning for caseload and assist other Case Managers with developing and updating support plans and assessments.
  • Ensure timely completion of required documentation for every client contact in accordance with agency policy and regulatory guidelines.
  • Facilitate groups on topics such as wellness, leadership, housing stability, employment, education, relationships, and healthy decision-making.
  • Coordinate group scheduling and maintain group notes binders; access outside providers and resources for client-centered groups and training.
  • Develop and supervise client activities and groups both on-site and off-site.
  • Provide crisis intervention as needed.
  • Assist and encourage clients with activities of daily living (ADLs), travel, appointments, and shopping.
  • Liaise with other departments and outside agencies to enhance service delivery.
  • Promote community services, assist with determining eligibility, and facilitate enrollment in programs (e.g., benefits, veterans services, education, volunteerism).
  • Maintain community involvement to identify needs and strengthen relationships with community resources and individuals.
  • Attend staff meetings, supervision, and training as required.
  • Interact with clients, families, and staff with care and respect, considering cultural differences.
  • Report serious incidents and sensitive situations to supervisors immediately; complete incident reports per Bridge policy.
  • Conduct diligent searches and file missing person reports for unaccounted consumers according to policy and procedure.
  • Perform any other duties as assigned.
QualificationsQualifications, Skills, and Knowledge:
  • Bachelor's degree required; Master's degree preferred.
  • Background in supportive residential programs and/or experience with individuals facing serious mental illness and homelessness.
  • Experience in crisis intervention and managing trauma-triggered behavior.
  • Willingness to obtain CPR, First Aid, AED, Crisis De-escalation, and Naloxone training.
  • Excellent time management and organizational abilities.
  • Strong team player with the capacity to demonstrate stress resistance, flexibility, creativity, and openness to feedback.
  • Ability to thrive in a high-stress, fast-paced environment.
  • Preferred knowledge or experience in vocational and educational resources, along with a background in coordinating and facilitating groups and activities.
  • Bilingual Spanish is highly desired.
