Job Description:

NYC Health + Hospitals Office of Behavioral Health seeks a visionary and dynamic leader to join our Office of Behavioral Health Care Management Strategies. The Senior Director of Care Management will spearhead the design, implementation, and oversight of innovative care management programs aimed at enhancing patient well-being, addressing social determinants of health, and optimizing healthcare outcomes. This pivotal role focuses on reducing acute care utilization while enhancing ambulatory care services across our health system.

GENERAL TASKS AND RESPONSIBILITIES WILL INCLUDE:

Behavioral Health Community Health Worker (CHW) Program Oversight:

  • Supervise operations of CHWs across various settings including inpatient, outpatient, and emergency departments.
  • Develop and refine CHW workflows, integrating tailored patient and staff feedback to optimize clinical service delivery.
  • Manage CHW recruitment and training, ensuring compliance with NYS Medicaid, OMH, and other regulatory bodies.
  • Monitor and evaluate training programs like Compass Rose, ensuring compliance and alignment with patient care goals.

Quality Improvement:

  • Engage in strategic partnerships with facility and central office stakeholders to improve health outcomes and meet regulatory standards.

Care Management Program Collaboration:

  • Lead collaborations with Managed Care Organizations (MCOs) to enhance care management strategies.
  • Exchange critical care management updates with the H+H Managed Care Team, disseminating essential information to CHWs and social work teams.
  • Oversee the CTI Pathway Home service deliverables, ensuring effective performance and outcomes.
  • Coordinate with CBC Leadership to monitor and address performance under contracted measures.
  • Recommend performance enhancements and corrective actions to improve service delivery and contractual compliance.

Community and Housing Initiatives:

  • Expand referrals to Health Homes and Health Home+ services through active collaboration with Community Care.
  • Integrate Housing for Health (H4H) services within the CHW service portfolio, managing patient engagement and housing coordination efforts.
  • Other related duties as directed.

Minimum Qualifications:

1. A Master's degree from an accredited college or university in hospital administration, health care planning, business administration, public administration or an approved related program and seven (7) years of progressively responsible experience in health and medical service administration, public administration, personnel and labor relations, finance or appropriate functional discipline with an emphasis on planning, liaison and inter-organizational relationships, or related administrative or managerial functions; or

2. A Baccalaureate degree from an accredited college or university and eight (8) years of progressively responsible experience in health and medical service administration, public administration, personnel and labor relations, finance or appropriate functional discipline with an emphasis on planning, liaison and inter-organizational relationships, or related administrative or managerial functions; or

3. A satisfactory equivalent combination of education, training and experience. However, all candidates must have a minimum of a Bachelor s Degree.

Department Preferences

  • Proven leadership in healthcare management, particularly in behavioral health settings.
  • Strong background in program development, quality improvement, and managed care collaborations.
  • Excellent communication and interpersonal skills, with the ability to lead diverse teams and forge multi-stakeholder partnerships.
  • In-depth knowledge of regulatory requirements related to Medicaid, OMH, and HEDIS measures
