DISTINGUISHING FEATURES OF THE CLASS: The work involves performing various clerical functions necessary for the filing, retrieval, storage and retention of medical records at the Erie County Medical Center Corporation (ECMCC). The incumbent organizes and maintains medical records, reviews records for accuracy and completeness in accordance with established policies and trains new staff. Work is performed under the direct supervision of an administrative technical or clerical employee. Supervision may be exercised over lower-level clerical staff. Does related work as required.
TYPICAL WORK ACTIVITIES:
Organizes and maintains the contents of medical records in accordance with prescribed procedures and policies and ensures completeness of records;
Analyzes medical records to ensure that all required information for Joint Commission, New York State Department of Health and Medical By-law requirements is documented and notes deficiencies for missing data to be completed by attending physicians or other staff;
Pulls and locates deficient records for medical staff to complete; reviews records to verify deficiencies for missing data are completed as required;
Ensures transcribed reports are attached to the correct patient/visit in the Electronic Medical Record; corrects any reports that fail or have misinformation;
Audits auto-fax feature of electronic signature to ensure that signed reports are faxed to the appropriate and necessary physicians;
Trains and orients new Health Information Management Clerks;
Trains new physicians on the dictation system including electronic signature for reports and setting up dictation ID;
Audits signing queues;
Completes weekly delinquent lists for physicians, operative reports, cardiac catheterizations and angiograms;
Prepares weekly letters for medical staff;
Scans, indexes and performs quality checks on scanned documents;
Receives, verifies and prepares records subpoenaed for court hearing or trials;
Completes birth and death certificates;
Enters and prepares records for New York State Disability claims.;
Answers telephone, attends to walk-in requests at Medical Records reception and performs other clerical duties requested by higher-level staff.
FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of modern filing and medical record keeping procedures including but not limited to paper, microfilm, scanned and electronic records; thorough knowledge of assembly order for medical records; good knowledge of laws and regulations governing the completion of medical records; good knowledge of laws and regulations regarding release of protected health information; good knowledge of office and medical terminology, procedures and equipment; ability to work with an electronic medical record system; ability to establish and maintain effective working relationships with a diverse constituency; ability to supervise lower-level staff; ability to communicate effectively, both orally and in writing; ability to maintain a variety of medical records neatly and accurately; ability to understand and follow detailed oral and written instructions; high degree of accuracy; industry; dependability; capable of performing the essential functions of the position with or without reasonable accommodation.
MINIMUM QUALIFICATIONS:
Graduation from a high school or possession of a high school equivalency diploma and one (1) year of experience in a Medical Records/Health Information Management Department in a health care setting.
