Job Type Full-timeDescriptionGreater Nashua Mental Health is looking for Senior Health Mentor to join our Integrated Care Team! The Senior Health Mentor leads a team of health mentors in implementing the InSHAPE (Individual Self-Help Action Plan for Empowerment) Wellness Program by providing administrative support as well as coaching and guidance on fidelity to the program model. Carries a caseload of InSHAPE participants and provides support and education related to physical fitness, nutrition, and wellness to assigned caseload. Provides individual and group interventions to promote and support healthy lifestyle habits.Duties & Responsibilities:
  • Delivers community-integrated InSHAPE Health interventions that promote and support healthy lifestyle habits through individual and group-based interventions to assigned caseload for a percentage of time per week as appropriate for position and as directed by supervisor.
  • Ensures documentation (including progress notes and program assessments) are completed within required timeframes for caseload.
  • Supports the success of the InSHAPE program through marketing and recruitment of referrals within the agency.
  • Oversees referrals to the program, working with Health Mentors and referring staff to ensure all enrollment paperwork and medical clearance has been completed in a timely fashion. Maintains an active roster of program participants as well as a wait list if necessary.
  • Takes the lead in organizing team in providing quarterly celebration/recognition events and new program activities as appropriate within the framework of the InSHAPE model.
  • Develops relationships with community partners to enhance program offerings.
  • Acts as a resource to the team, providing guidance around program fidelity, tobacco cessation treatment, service delivery, documentation standards, and agency policies and procedures through participation in group supervision and one-on-one meetings with Health Mentors as directed by Director of SUD Services and Integrated Care.
  • Provides administrative support to ensure program materials (e.g. handbook, assessments, etc.) are kept up to date.
  • Supports Director of SUD Services and Integrated Care in monitoring fidelity to program through chart audits and program fidelity reviews.
  • Supports Director of SUD Services and Integrated Care in hiring and training new Health Mentors.
  • Participates in regularly scheduled state and national InSHAPE conference calls.
  • Performs other duties or special projects as required or as assigned.
RequirementsQualifications:
  • Bachelor's degree in Health Science, Social Work, Rehabilitation Psychology, Education or related field
  • 1 to 2 years related work experience
  • Experience and/or training in motivational interviewing and motivational strategy building.
  • Experience and/or training in tobacco cessation treatment interventions.
  • Commitment to being physically active and promoting the holistic benefits of good health.
  • Commitment to reducing health disparities and social stigma towards mental illness.
  • Ability to coach/train effectively.
  • Fluency in Spanish strongly preferred.
  • Strong organizational and time-management skills.
  • Knowledge of Microsoft Office products (Email, Word, EXCEL).
  • Strong collaboration skills, and an ability to work effectively both independently and as part of a team.
  • Comfortable in a fast-paced environment with frequent deadlines; high energy, self-motivated and proactive.
  • Excellent communication and interpersonal skills.
  • Ability to multitask, and be flexible with regard to workload and assignments.
  • Ability to exercise judgement.
  • Must possess a valid driver's license and automobile with liability insurance in the amount required by agency policy
