Hourly range: $66.22 - $89.65DISTINGUISHING FEATURES OF THE CLASS: The work involves coordinating clinical services of a transplant unit on a twenty-four (24) hour basis and providing direct clinical services to patients requiring transplant services at the Erie County Medical Center Corporation (ECMCC). The incumbent coordinates multidisciplinary activities on the unit, supervises personnel and manages the delivery of quality care. The incumbent works in collaboration with members of the health care team to provide, enhance or improve health care delivery by evaluating patients' response to medical and nursing care, developing best practices and implementing evidenced-based standards of care, and assessing and addressing patient and staff educational needs. This class differs from other Nurse Practitioner classes by virtue of its responsibility to serve as Unit Manager of a transplant unit and increased work responsibilities specific to quality assurance/performance improvement. Work is performed under the general direction of a licensed physician and under the direct supervision of the Vice President of Transplantation and Renal Care. Supervision is exercised over professional, paraprofessional, clerical and support staff. Does related work as required.TYPICAL WORK ACTIVITIES:Manages the delivery of patient care in a transplant unit, including, but not limited to: patient assessment, care planning, health teaching, discharge planning and quality improvement activities;Coordinates and directs patient care activities and nursing activities for transplant services with those of other disciplines and departments;Oversees operations of designated areas to ensure optimal department functioning, including, but not limited to minimizing operating cost, maintaining adequate staffing and managing supply utilization;Assesses patients who are candidates for kidney and/or pancreas transplant and candidates for living kidney donation;Complete patient history and physical; initiate orders, interpret results, and develop overall plan of care;Participates in performing multi-disciplinary clinical rounds on a routine basis;Identifies and documents critical deviations in patient condition; initiates treatment orders and notifies physician;Manages and coordinates organ offers and transplant of cadaver kidneys and pancreas;Evaluates productivity and makes recommendations for the development of productivity standards;Oversees provider scheduling, billing and charge procedures; recommends initiatives to enhance revenue;Participates in interviewing and hiring, completes evaluations, approves overtime, monitors time and attendance, conducts staff meetings, identifies educational needs and job performance issues and ensures technical competency of staff;Provides supporting evaluation feedback for indirect reporting personnel supporting the defined service areas;Develops and implements staff orientation plans and completes required orientation documentation;Educates patients and caregivers regarding transplantation;Ensures sufficient Quality Assurance/Performance Improvement (QAPI) documentation is available to identify problems, identify opportunities for improvement, resolve issues and provide process loop closure; works in collaboration with higher-level administrators in assisting QAPI as required;Oversees the abstraction, input and submission of transplant clinical data into applicable electronic software applications as required;Develops corrective actions plans for improving the nursing practice and related non-clinical practices as it relates to transplant services; utilizes various data sources in developing corrective actions;Develops work schedules, plans and assigns work assignments, evaluates employee performance, assists in disciplinary measures and monitors staff compliance with required credentials; participates in employee education and development initiatives;Develops programs to meet changes in the nursing practice;Assists with the completion of budget and reviews variances as directed by supervisor;Coordinates special projects and participates in committees, multi-disciplinary care programs, task forces, research opportunities and poster/research presentations as assigned;Participates in and organize meetings, in-services, trainings, teaching/student engagement opportunities and public outreach opportunities.FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of the application of current nurse practitioner, professional nursing and transplant care principles, practices and techniques; thorough knowledge of applicable laws, rules, regulations and accreditation standards as relate to the nurse practitioner practice, professional nursing practice and transplant care; thorough knowledge of advanced health assessment, physical examination and diagnosis procedures; thorough knowledge of the current developments, literature, research studies and sources of information in the field of transplant services and critical care; thorough knowledge of quality assurance/performance improvements (QAPI) principles, practices and terminology as they relate to the nurse practitioner, nursing practice, transplant care and related areas; good knowledge of transplant standards as outlined by the Joint Commission, Centers for Medicare and Medicaid Services (CMS), New York State Health Department, United Network for Organ Sharing (UNOS), Organ Procurement and Transplantation Network (OPTN) and Connect Life; ability to recognize and resolve transplant and related critical care issues; ability to plan and evaluate care provided in a transplant services setting; ability to work effectively on a multi-disciplinary team; ability to abstract transplant clinical data and utilize such data for making clinical and non-clinical process improvements in transplant and related critical care areas; ability to supervise and direct subordinate staff; ability to work nights or weekends; ability to rotate an on-call schedule; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with a diverse constituency; ability to utilize a variety of electronic software applications; sound professional judgment; capable of performing the essential functions of the position with or without reasonable accommodation.MINIMUM QUALIFICATIONS:Possession of a Master's Degree* in Nursing and eight (8) years of experience as defined below; either:* Five (5) years of experience as a Nurse Practitioner in any specialty practice area and three (3) years of registered professional nursing experience in a transplant unit; or* Four (4) years of experience as a Nurse Practitioner in any specialty practice area and four (4) years of registered professional nursing experience in a transplant unit;* An equivalent combination of training and experience as defined by the limits of (A) and (B).SPECIAL REQUIREMENTS:Possession of a license and current registration to practice as a Registered Professional Nurse in New York State at time of appointment and maintenance throughout duration of appointment;Possession of a license and current registration to practice as a Registered Professional Nurse in New York State at time of appointment and maintenance throughout duration of appointment; andPossession of a current New York State Education Department license as a Nurse Practitioner that specifies specialty area of practice at time of appointment and maintenance throughout duration of appointment; and Possession of a Nurse Practitioner certification as issued by the American Nurses Credentialing Center (ANCC) or American Association of Nurse Practitioners (AANP) at time of appointment and maintenance of certification throughout duration of appointment; andPossession of a current Federal Drug Enforcement Administration (DEA) number issued by the United States Department of Justice Drug Enforcement Administration at time of appointment and maintenance throughout duration of appointment;
