This position is responsible for providing Case Management and individual living counseling or other community living services to people who have disabilities and elders. The Service Adviser is an advocate to the individual to define and direct their own personal assistance needs and services. This position works in the Independent Provider and Personal Choice programs.

The Independent Provider, or IP Program, is a consumer-directed program designed to provide in-home services and supports to adults with disabilities and elders. Consumer-directed programs allow the person to have the right to self-determination, to live independently and to be integrated fully into the community. A person chooses self-direction because it affords choice, flexibility, and control. The consumer needs to be approved for Medicaid Long Term Care Services (LTSS).

The Personal Choice is a self-directed care program for those who want the choice and control over those who provide care to the recipient. Recipients, or their representative, choose who they wish to hire as caregivers. A budget is allotted based on the degree of care the recipient requires after assessments have been made upon approval into the program

Responsibilities:

Assess for initial eligibility for Long Term Services and Supports (LTSS)Conducts screenings for eligibility and acceptance into the Personal Choice program or Independent Provider Model - IP Program Provide ongoing training and assistance to participants and or their representative.Conducts participant assessments and reassessments to identify individuals needs and desires through formal tools and various personal contacts, including face-to-face interactions.Schedules and monitors quarterly meetings and other meetings to review and asses participant needs.Monitors documentation in participant records to assure compliance with department and other State Regulators.Maintains each case record in compliance with EOHHS regulations.Ongoing contact via telephone on a monthly basis and conducts face to face meetings.Coordinate yearly visits based on the program regulations.Reports critical incidents within the required time-frame.Informs participants and their families of available community services and refers participants to agencies.Coordinates ancillary services provided in the community as needed/appropriate.Monitors service outcomes, with the interdisciplinary team, individual, and other stakeholders as needed.Serves as a liaison between individuals, service providers, family, guardians and social caseworkers.

Qualifications:

Bachelors Degree required in Human Service or other Health related fieldMinimum of two (2) years in related service area.Strong advocacy and community liaison abilities Effective written, verbal and nonverbal communication skills required.Valid Driver's License and personal vehicle required.
