Our nurturing communities provide well-maintained, affordable apartment housing for low-income elderly and disabled individuals in a comforting family atmosphere.This is a Full Time day shiftSenior Living at Redeemer Community Position Summary:Coordinate and monitor supportive services and case management for individual residents. Establish a comprehensive and ongoing person-directed life plan that promotes wellness, personal responsibility, use of technology, exercise, activities and socialization, and community engagement, to enhance the long-term quality of life for residents. Provide health and wellness group educational opportunities that help residents remain independent and age in place as long as possible.Duties:Promote wellness and educational group activities for all residents, coordinating at least one such activity per month. Use accepted disclaimer language on flyers, sign-in sheets and other communications.Develop contacts with community services and agency resources for resident referrals. Coordinate information on services and assist residents, families, and staff in becoming educated in/and linked to the available services.In cases where services are not available to residents, work with local agencies, civic groups, businesses and churches to develop specific services, including working toward receiving donations from different sources.Promote volunteerism for mutual assistance and in coordination of resident social events. Plan volunteer recognition event at least annually. Provide problem solving through one on one contact, home visits, family contact and group meetings. Encourage residents to be proactive in meeting their own social, psychological, and physical needs and being involved in decision-making process. Facilitates meeting of needs in an acute situation and works to find more long-term solutions so as not to create dependence.
