We are seeking qualified applicants for a Service Coordinator/Case Manager position, who will work with adults/children with Intellectual and/or Developmental Disabilities (I/DD) who are eligible for case management services.Responsibilities:
- Provide case management services for adults/children with I/DD and their families/support team
- Assist individuals, families/support team in accessing services and resources available
- Developing, coordinating, and monitoring of Individual Support Plans (ISPs)
- Work with partnering agencies, community organizations, school/educations programs, etc. to provide comprehensive service coordination to supported individuals
- Serve as an advocate/liaison to assist individuals in obtaining and accessing necessary services including obtaining referrals or finding resources in the community
Qualifications:
- Any combination of the following education/and or experience requirements
- Bachelor's degree in behavioral science, social science, or a closely related field; or
- Bachelor's degree in any field and one (1) year of human services related experience; or
- Associate degree in behavioral science, social science, or a closely related field and two (2) years of human services related experience; or
- High school diploma/equivalent and three (3) years of human services related experience
- Good organizational and time management skills
- Excellent verbal and written skills/communication
- Valid driver's license
- Knowledge of the Oregon Administrative Rules (OARs) is preferred
