Job Type Full-time Description We are currently hiring for Service Coordinator in our Chenango/Broome/Cortland County Region. As a Service Coordinator at Venture Forthe, you will be responsible for providing primary assistance to the waiver applicant/participant in gaining access to needed waiver and Medicaid State Plan services. Service Coordination (SC) has two basic components: Initial and Ongoing Service Coordination. This job description is mostly based in Ongoing Service Coordination although Service Coordinators may be requested to author and build an Initial Service Plan. Ongoing and Initial Service Coordination tasks are outlined in the current manual or application for each Waiver program noted. Under the Traumatic Brain Injury (TBI) Waiver Program or the Nursing Home Transition and Diversion (NHTD) Waiver Program, the participant is the primary decision-maker in the development of the service plan. All goals, selection of supports and individual service providers are initiated via communication with the identified Service Coordinator. The Service Coordinator is responsible for assuring that the Service Plan is implemented appropriately and supporting the participant to become an effective self-advocate and problem solver. Together they work to develop and implement the Service Plan, which reflects the participant's goals. The ultimate responsibility for assuring that the Service Plan is appropriately implemented rests with the Service Coordinator. Services may include Medicaid State Plan services, non-Medicaid federal, state and locally funded services, as well as educational, vocational, social, and medical services. The goal is to increase the participant's independence, productivity and integration into the community while maintaining the health and welfare of the individual. Service Coordinators report directly to SC Leadership. SC Leadership does make the determination of where cases may be allocated. This full-time SC position is for 20-22 cases and is considered to be completed somewhere between 32-40 hours per week. Requirements - An eligible applicant for this position would be required to have a valid NYS Driver's license with the ability to travel to where their applicants/participants live -Has Service Coordination experience working with the TBI and/or NHTD Waiver programs - Vetted ability to successfully work independently to submit timely documentation required to complete SC job tasks. - Vetted ability to successfully manage community resources required to build Service plans and assist with SC training and resources for the team.
