LOCATION: Louisville, KY (South Oaks)

STATUS: Full Time, Hourly, Non-Exempt

PROGRAM: HUD Housing

REPORTS TO: Community Administrator

JOB SUMMARY AND QUALIFICATIONS:

Service Coordinator will be responsible for ensuring maximum utilization of community resources by residents of Volunteers of America Mid-States' housing programs for the elderly or disabled. This utilization of resources is aimed at maximizing residents' length of stay within the housing programs by stabilizing and/or enhancing quality of life.

WHAT YOU SHOULD HAVE FOR THIS ROLE:

This position requires a Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling.

The position requires a reasonable combination of the following skills and knowledge base: the aging process, elder services, disability services, eligibility for and procedures of Federal and applicable State entitlement programs, legal issues, drug and alcohol abuse, poverty, and mental health issues. At least two years experience in social service delivery with senior citizens or non-elderly disabled persons is preferred.

RESPONSIBILITIES:

1. Complete assessments for any residents requesting services.

2. Develop case plans based on assessments; update case plans periodically and record progress in case notes after each contact with resident or family members; document all case management activities.

3. Identify community resources and develop cooperative relationships with other social service providers.

4. Monitor the on-going provision of services and keep provider informed as to progress of the residents as appropriate.

5. Educate residents on service availability, application procedures, clients' rights, etc.; provide advocacy as needed.

6. Develop and maintain a directory of service providers for project staff and residents to use.

7. Submit HUD and other statistical reports accurately and in a timely fashion.

8. Organize and implement special projects designed to improve the social networks and natural helping systems among residents.

9. Represent agency programs and policies accurately through community involvement.

10. Utilize volunteers to provide support programs when appropriate.

11. Attend supervision and training sessions as scheduled.

12. Performance quality improvement (PQI) duties as assigned by supervision & PQI committee.

13. Attend monthly VOA Mid-States Coordinator teleconference, regional Service Coordinator teleconference, and other pertinent meetings/trainings designated by the HUD QA Administrator.

14. Actively participate in HUD QA activities throughout the year with the knowledge of the HUD QA, Administrator will share information with the supervisor for this position as part of the performance review process.
