New Horizons is the largest mental health and addiction recovery provider on the Treasure Coast (and beyond), serving 15,000 children and adults annually through inpatient crisis services, 24-hour help line and mobile response team, and outpatient programs conveniently located across Indian River, Martin, Okeechobee and St. Lucie counties. In addition, New Horizons assists 22,000 students in area schools, and we work closely with the courts, law enforcement, jails, and hospitals to help improve the health of individuals and the quality of life in our community.

New Horizons is a nonprofit agency seeking a caring and compassionate Service Engagement-Community Transition Specialist

to direct the court approved treatment plan for Mental Health Court participants who are not eligible for traditional case management services. As a SECTS , you Will engage the client, provide direct care until other traditional services can be engaged, assist them with obtaining transitional housing and transitioning into permanent stable housing and prepare them for discharge from MH Court by preparing an aftercare plan.

As a Service Engagement-Community Transition Specialist your responsibilities will include:

* Provide direct services until benefits are obtained and traditional case management becomes available

* Coordinate referrals to necessary services such as psychiatric and medical care. Referral for drug screens and other resources/services

* Communicate necessary information to the on-going treatment provider

* Assess client's ability to attain permanent, independent housing. Refer to housing programs such as Shelter Plus Care, HUD or other available grants in order to establish permanent housing, if they are unable to afford such housing on their own

* Communicate with Mental Health Court

* Ensure that a continuum of services and aftercare is available to the client following his graduation from Mental Health Court

* Provides transportation as needed, while pursuing alternative transportation

Requirements

New Horizons will need you to have the following qualifications:

* Bachelor's degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field

* One (1) year full time or equivalent experience working with adults experiencing serious mental illness

* Valid and current Florida Driver's License required with no more than 6 points in the past 3 years
