About AOASCC: The AOASCC is an independent non-profit organization that serves older adults, individuals with disabilities, and caregivers. Our mission is to champion and support these groups, enabling them to remain independent and engaged within their communities.

We are seeking a Service Navigator (Social Worker) Empowering Older Adults and Caregivers. This role is crucial to our mission and we are excited to welcome a new team member who shares our passion for supporting older adults and caregivers.

As a dedicated Service Navigator at the Agency on Aging of South Central Connecticut, you'll be at the forefront of supporting older adults and caregivers.

Responsibilities:

As a Service Navigator at the Agency on Aging, you will be vital in going beyond paperwork and processes to empower older adults and caregivers. Your efforts will help them navigate the complexities of aging-related services, making a real difference in the lives of our community members through:

  • Complex Enrollments: Guide clients through intricate enrollment processes for various services and programs.
  • Person-Centered Planning: You'll work closely with individuals to create personalized care plans that address their unique needs and preferences, fostering a sense of collaboration and connection.
  • Accessing Benefits and Support: You'll help clients access available benefits, resources, and support services.

Qualifications:

  • A Bachelor of Social Work is required, and a Master of Social Work (MSW) degree is strongly preferred.
  • Three (3) years of Case Management Experience
  • Strong communication and interpersonal skills.
  • Empathy and compassion for older adults and individuals with disabilities.
  • Ability to work independently and as part of a team.
  • Familiarity with community resources and benefit programs.
  • AIRS certified or eligible to sit for certification AIRS Certification within 120 days of hire
  • SHIP/CHOICES certified

Compensation details: 49000-50000 Yearly Salary
