Job DescriptionThe Family Preservation Family Skill builder is an essential member of the service team assigned to each case. The Skill Builder provides support services to the family, at the direction of the Family Preservation Therapist/Case Manager. The goal is to provide a full range of services to the family to prevent out of home placement of one or more children. The Family Preservation Family Skill Builder utilizes the strengths of the family to resolve issues and maintain the family.
  • The Skill Builder is an essential member of the service team assigned to each case.
  • Provides intensive, in-home family support services assisting with case plan goals.
  • Assists in providing support services as identified through various assessments, including the bio-psycho-social assessment, family conferencing (case planning), service delivery and day-to-day practices.
  • Refer clients to community services and assist in arranging appointments as needed.
  • Assists in researching possible relative and kin placement in the event out of home placement of the child or children would occur.
  • Participates in family conferencing (case planning), family meetings, and in the team-decision making process.
  • Support case plan goals through teaching, role modeling, and demonstration
  • Provides direct in-home services including but not limited to parenting skills, life skills, child development, and behavior management.
  • Becomes familiar with and utilizes Strengthening Families curriculum.
  • Schedule and supervise/monitor families' interactions and transport children and families.
  • Provides 24/7 crisis intervention for referred families including on-call rotation.
  • Document all contacts with families and collaterals on agency database and complete all necessary Saint Francis Ministries forms and reports according to timelines.
  • Attends court hearings and provides court testimony as required.
  • Assists Therapist/Case Manager in ensuring clients' rights are protected.
  • Reports critical incidents to DHHS verbally within 12 hours, written within 24 hours.
  • Reports critical and unusual incidents to Saint Francis Ministries Risk Management Department, and Therapist/Case Manager
  • Has working knowledge of and adheres to the Family Preservation contractual requirements outlined by the State of Nebraska DHHS
  • Is knowledgeable of and follows all safety procedures.
  • Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures (SOP's), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state and local rules, statutes, and regulations.
  • Ensures clients' rights are protected.
  • Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
Requirements
  • Bachelor's Degree in Human Services field, psychology, family sciences
  • Prefer 2 years' experience in family-based services, child development, child welfare.
  • Must be 21 years of age.
  • Must pass drug screen, Nebraska Child Abuse and Neglect Central Registry Clearance check.
  • Must have a valid NE driver's license, acceptable driving record and reliable transportation.
  • Lifting requirement of 30 lbs.
