Overview:

Under the supervision of the program staff, performs a variety of tasks and services to assist the provision of care and comfort for patients; provides care appropriate to the age of the patients served; reports observations; contributes to the efficient functioning of the unit; all duties are performed in accordance with established policies, procedures, standards and guidelines.

Qualifications:

Education:

  • GED/High School Diploma required
  • Successful completion of unit-based orientation and clinical competency validation tool.

Licensure:

  • Certified as a nursing assistant, preferred
  • CPR certification required within 30 days of hire and maintained throughout employment.

Other Knowledge/Skills/Abilities:

  • Data entry/management required; scheduling experience preferred; Microsoft Office Suite experience preferred; Hospital EMR experience preferred; Basic office skills required.
Responsibilities:

1. Performs and documents nursing care measures to meet the physical needs of patients as directed by the registered nurse. Illustrative tasks and expectations include, but are not limited to; provides hygiene and comfort measures, assuring privacy and preventing undue exposure. Assists with patients activities including ambulation, transfers; uses proper body mechanics and positioning techniques. Serves and collects food trays and nourishments. Provides assistance to bathroom. Assists with admission, transfer and discharge of patients.

2. Performs and documents monitoring procedures; observes and reports the patient response. Illustrative tasks and expectations include, but are not limited to; vital signs (BP and TPR), weights, and unusual conditions and/or change in patient behavior or appearance.

3. Performs therapeutic measures/procedure, observes and reports changes, deviations from normal.

4. Maintains safety and security of patients/family/self/others, observing safety precautions and promoting a sense of security and wellbeing. Illustrative tasks and expectations include but are not limited to; follows standard precautions, washes hands between patients, reports equipment malfunctions, assists in keeping patient care area/corridors free of clutter, cleans fluid spills, use proper body mechanics in moving, transferring and transporting patients, and interacts with patients/visitors/others in a courteous and respectful manner.

5.Performs additional unit-specific tasks/care elements for which they have received specific instruction and demonstrated competence.

6. Performs scheduling of patients for transportation, daily appointments and physician visit appointments. Records attendance, performs billing activities, data and file management, manages patient charts, manages physician timesheets and physician scheduling. Manages telehealth and IT connections, inventories and orders supplies/resupplies. Participates in community relations activities including marketing, educational sessions, health fair/expos. Maintains records and collects data as directed by program staff for quality, safety and performance improvement.

7. Performs other related work as required or requested.
