To us, it's a privilege and our calling.Benefits/Perks:* Includes accrued vacation days, sick days, and holidays.* Participation in 403 (b)* Voluntary Benefits: Short Term Disability, Life Insurance, Cancer Insurance, Pet Insurance* Discounted membership: Plum Benefits* Pay for work-related eligible commuter expenses with pre-tax dollarsResponsibilitiesResponsibilities:* Assess and evaluate the health care needs of patients and families with consideration regarding physiological, psychological, social and environmental factors.* Identify complete, accurate, and logical OASIS data and appropriate sequencing of diagnoses.* Develop a care plan based on patient needs, physician orders, nursing assessment parameters and identified goals that are respectful of patient, family, community, and agency resources.* Implement the care plan and utilize nursing judgements to modify plan as needed.* Evaluate for and promote a safe environment for the patient and complies with JCAHO National Patient Safety Goals.* Meet requirements for OASIS lock dates, re-certifications, and discharge timeline.* Communicate with other health care professionals both internal and external to the organization in a manner that assures care plan coordination as well as continuity of care.QualificationsQualifications:* Current NYS registration to practice professional nursing* 1 year of general nursing experience with current home health, med/surg, or critical care experience* Driver's license, insurance, and transportation for work as requiredEducation:* Associate's degree in Nursing from an Accredited College/UniversityJob Type: Full-timeSalary: $114,000 to $118,000 per year
