Job Description:
The Case Manager will work as part of the social service team. The Case Manager is a non-exempt employee; a Bachelors' Degree is preferred, a candidate with a high school diploma and five years of experience working with seniors would be considered. The Case Manager reports to, and is closely supervised by, the Social Worker II or Clinical Coordinator or Director of Clinical and Social Services, and works collaboratively with the Building Manager. The Case Manager will develop 1:1 relationships with the residents and will provide a comprehensive range of case management services to the residents of WSFSSH, enabling the residents to live as safely and independently as possible. Direct Services to Residents Assist residents in obtaining and maintaining appropriate medical and mental health treatment. Assist residents in obtaining and maintaining entitlements. Assist residents in obtaining and maintaining home care. Provide care coordination as needed between community service providers. Assist residents in establishing and/or maintaining family and social relationships. Through in-house and community referrals, assist residents in participating in socialization activities. Assist residents with budgeting, income maintenance etc. to ensure on-time payment of rent. Follow up with resident and other service providers during out of facility stays such as hospitalizations and/or short-term rehabilitation placements. Under supervisor's guidance, participate in discharge planning as assigned Escort residents to appointments as needed. Provision of crisis intervention services as needed. Provision of recreational group work services for residents. Additional Responsibilities Maintain professional roles and boundaries Maintain thorough case records, including case notes, support plans and psychosocial histories within the time frames required by WSFSSH and the contract/regulatory agencies where applicable. Work cooperatively with other staff members Share information about resident progress, needs, and problems with other staff as appropriate, guided by the WSFSSH Confidentiality Policy Develop and maintain competency in software and data bases as required Attend training sessions and conferences as required for enhancement of job skills Implement emergency procedures as necessary Attend supervision meetings, team meetings and staff meetings Maintain awareness of and sensitivity to issues of Diversity, Equity and Inclusion Other duties as assigned Qualifications BSW or BA in human services field preferred HS Diploma with at least 5 years related experience will be considered Fluency in written and oral English required Experience with older adults and people with mental illness and/or substance use issues preferred Will work Monday - Friday 8:30a-4:30p for the first few months to train.
