Participates as a contributing member of the team and brings authentic self to discussions regarding all aspects of the work.Demonstrates ability to build and maintain effective relationships.Contributes positively to an inclusive workplace.Organizes and disseminates paperwork, e-file court documents, reports and letters as required.Accepts and processes clients requesting services.Tracks clients, caseloads or service programs and assists in maintaining case records and reports.Schedules and coordinates trainings and groups.Work closely with APS Social Worker in performing paraprofessional work.Serves as APS liaison with hospitals, nursing homes, community and judicial systems in coordinating and processing the paperwork necessary for mental health, alcohol and drug commitments, and guardianship and protective placements.Meets with consumers to serve and explain upcoming court process.Enters case data and critical incident reports into Avatar database.Secures court dates and coordinate transportation for consumer to attend hearing when needed.Monitors and ensures that appropriate reports and documents are ready for APS Social Worker when attending court hearing.Maintains confidentiality of client-related information, maintain respectful treatment of clients, and adhere to the La Crosse County Standards of Conduct in the Administrative Code (3.08).Maintain skill and competence levels as policies and regulations change by attending department/staff meetings and by participating in continuing training as required and appropriate.Knowledge, Skills and Abilities•Knowledge of agency services and community resources.•Knowledge of needs and behavioral patterns of client/customers being served.•Ability to communicate effectively orally and in writing.•Ability to use computer, County e-mail, internet and certain computer software programs.•Ability to work as part of a service delivery team.•Ability to maintain courteous, cooperative and respectful working relationships with other employees, management and the community.•Ability to be prompt and regular when reporting to work.•Ability to interview people, extract information and make good, independent judgments.•Ability to accept, adapt and adjust to changes.•Ability to participate in meaningful training or educational opportunities provided by the agency, State or other resources.•Ability to compile data/documentation and provide accurate and understandable reports.•Ability to participate in, and appropriately use, supervision.•Ability to plan and organize work effectively.•Ability to effectively work with diverse populations.•Ability and willingness to work hours outside a standard work week.•Basic knowledge and understanding of the aging process, physical and developmental disabilities, mental health and substance abuse issues and their effect on individuals and their families.•Basic knowledge of the statutory requirements and processes of a Chapter 51 detention, mental health and AODA commitments, and Chapter 54/55 protective placement and guardianships.•Requires experience working with persons with mental illness, chemical dependency, developmental disabilities and individuals in crisis.
