Newport Meadows Nursing and Rahabilitation Center is searching for an experienced Social Service Assistant to join our Long-Term Care team!
Long term care experience with the geriatric population is required for this position.
Job Type: Part Time, 4 days a week, can be flexible

Social Service Assistant - Job Responsibilities:
- Orients resident and family to nursing center, helping both adjust to new environment, and offers support as needed
- Completes social history and MDS assessments, and develops care plan, which identifies medically related social and emotional problems and needs with realistic goals and specific actions to be taken
- Conducts/participates in resident care conferences with resident, family, and appropriate staff
- Advocate and promote resident rights
- Receives complaints from residents and families, forwarding complaints to appropriate staff to ensure resolution. Documents complaints. Follows facility policies and procedures
- Performs other related duties, as assigned
Education & Qualifications
- Bachelor's degree from accredited school in Social Work or related field preferred.
- 1 year experience in long-term care is required
- A physical examination and/or evidence from a physician indicating freedom of communicable disease to be available upon employment.
- Must be in good physical and mental health.
- Must be knowledgeable of social services practices, procedures, laws, regulations and guidelines governing long-term care
- Must possess the ability to deal tactfully with personnel, resident, family members, visitors, government agencies/personnel, and the general public.
- Knowledge of elderly, ill and/or disabled.
- Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
- Ability to work independently or part of a group.
- Effectively communicate with others.
