Job Type Full-time Description Summary Manages the delivery of services to residents of The Orchards and their family members by performing the following duties. Works closely with resident and family to ensure all matters of resident care are coordinated from admission to discharge. Essential Duties and Responsibilities Reviews and prioritizes grid of Minimum Data Set (MDS) assessments with various staff members each day. Reviews charting and data regarding residents' psychosocial information, behaviors, and moods. Interacts daily with residents and their families to counsel, intervene, and report on action items (can be walk-ins or scheduled in advance). Coordinates admissions for the Orchards, including collecting and processing all referrals. Coordinates discharges from the Orchards. Makes needed referrals, transportation, doctors orders (with nursing) and family meetings as needed. Other Responsibilities Attends meetings with other staff to discuss residents and to participate in care planning, restorative care planning, discharge planning, utilization review, falls, Medicare XCS, deme, case management, alleged abuse investigations, and quality assessment activities and analysis. Attends other meetings as required or appropriate for position. Reviews Nursing documentation to assist in developing Resident Assessment Protocols (RAPS). Initiates and update care plans for residents in cooperation with the appropriate staff. Creates, reviews, or processes various forms and reports, such as Complaint concern reports, Hospice residents, Discharge summary, budget reports and census reports. Coordinates with nursing the signing of personal inventories to comply with regulations. Educates all employees on Abuse and Neglect Policies and Procedures, HIPAA, grievance/complaints, Resident rights, Psychological needs, and confidentiality upon hire in orientations and annually. Education and/or Experience Associates or Bachelors Degree in Human Services, Social Work or related field preferred . Requires at least five years of related experience and/or training in a geriatric care facility; or equivalent combination of education and experience preferred. Complete annual state mandated training requirements (Regular In-services as well as any external training). Prefer Language Skills Ability to read, analyze, and interpret common scientific and medical journals, financial reports, policies and procedures, or governmental regulations. Ability to write reports, business correspondence, and procedures. Ability to effectively present information and respond to questions from individuals or groups of residents, family members, physicians, therapists, employees, outside business contacts, Board members, and the general public. Reasoning Ability Ability to deal with several abstract and concrete variables to define problems, collect data, establish facts, and draw valid conclusions when assessing potential residents, prioritizing work, evaluating the need for financial subsidies, making recommendations regarding resident care, disciplining employees, and investigating situations that arise from residents, family members, supervisors, and employees. Ability to apply education and experience to employ standardized or innovative solutions to resident and employee issues. Ability to interpret non-verbal gestures, body movements, and facial expressions in communicating with residents. Computer Skills Uses computer to review notes from nurses regarding resident care and prepare MDS reports. Uses e-mail to communicate with others internally and externally. Individual should also have strong knowledge of spreadsheet and word processing software for creating reports and correspondence. Certificates, Licenses, Registrations License in Social Work in preferred. Other Skills and Abilities Skills specific to communicating with geriatric residents and their families are needed. May need additional skills with special-need geriatric residents, such as those with dementia, Alzheimer's, or those requiring advanced health care.
