Social Services Director Job Duties
- Conduct comprehensive assessments of residents' psychosocial needs and develop personalized care plans in collaboration with the interdisciplinary team.
- Provide emotional support, counseling, and resources to residents and their families to address their social and emotional well-being.
- Assist residents in accessing community resources and services to meet their individual needs and preferences.
- Facilitate family meetings and serve as a liaison between residents, families, and the facility to ensure open communication and understanding.
- Collaborate with medical professionals, including doctors, therapists, and social workers, to ensure the holistic care of each resident.
- Assist residents with discharge planning and coordinate the transition to home or other care settings when necessary.
- Maintain accurate and confidential resident records, ensuring compliance with all relevant regulations and policies.
- Stay updated with changes in regulations and best practices related to social services to enhance the quality of care provided.
Qualifications of the Social Services Director:
- Bachelor's degree in Social Work or related field.
- Previous experience in social services or a similar role in a Long Term Care setting.
- Strong communication and interpersonal skills to interact with residents, families, and staff effectively.
- Empathetic and caring nature, with a genuine desire to improve the lives of our residents.
- Knowledge of relevant regulations and laws related to social services in a healthcare environment.
- Ability to work collaboratively with a diverse team and maintain a positive working environment.
