SummaryThe primary purpose of this job position is to plan, organize, develop and direst the Social Services Program in accordance with the current federal, state and local standards, and regulations. Essential Duties & ResponsibilitiesTo perform this job successfully, an individual must be able to perform each essential duty and responsibility satisfactorily.
  • Completing an Initial Assessment of all new residents' psycho-social needs. Charting at least quarterly and as necessary according to the MDS schedule. Charting on restraints; charting on behaviors and interventions and other changes in status as necessary.
  • Assure that social service progress notes are informative and descriptive of the services provided and of the resident's response to the service.
  • Interview resident and family members to obtain social history for initial assessment and plan for care. Involve the resident and family in planning objectives and goals for the resident.
  • Knowing which residents vote and obtaining an absentee ballot for them; If not registered and resident wants to vote, contact clerk's office and get the resident registered and obtain absentee ballot.
  • Participate in community involvement related to the interests of the facility and the services and needs of the resident and family. Team Leader for Community Involvement Projects; including March of Dimes, Relay for Life, Alzheimer's, Memory Walk, etc.; heading up fundraisers, encouraging team participation, etc.
  • Conduct discharge planning, contacting the physician for a discharge order; faxing MARS to the physician so that current prescriptions can be called in to the residents choice of pharmacy; scheduling home health to set up care and the equipment/supplies which the resident may need for home care.
  • Awareness of appropriate resources and agencies which may be able to assist residents.
  • Knowledge of local and state programs which may be able to assist residents.
  • Ability to understand medical treatment objectives in order to explain them to the resident and family.
  • Assisting the resident and their families in dealing with grieving, family issues, and crisis situations resulting from or which may affect the resident's physical or mental health.
  • Contacting local hospitals, home health, hospice agencies and physicians for resident referrals. Working with MDS personnel in the review of all referrals; getting pricing on medication lists from the pharmacy; if a Medicare admission, contacting billing office to check Medicare days; Obtaining a physician for new admits if they do not have a local one; updating name plates of resident rooms to show new admit name; alerting all departments to new admissions.
  • Scheduling of interdisciplinary care plan meetings for all residents at least quarterly. Mailing of care plan invitation letters to families and notifying the resident of the meeting and inviting them to attend. Keep record which includes care plan meeting minutes and attendance.
  • Conducting in-services with all personnel on, but not limited to; abuse, resident rights.
  • Complete a monthly newsletter with the input from the Activities Director, and after Administrator's approval, mail out to all families, referral sources, physicians, Ombudsman, local political leaders; Place a copy of the newsletter in every resident room, and extra copies in the facility lobby for visitors.
  • Shop one day a week for residents.
  • Assist with activities and special events as needed.
  • Plan and organize special events for holidays.
  • When a resident passes away, always send a Sympathy card and grief booklet to the family; attending services on behalf of the facility.
  • Arrange psychiatric consults as needed.
  • Assist in developing a written plan of care for each resident that identifies the problem and needs of the resident and the goals to be accomplished for each problem or need identified.
  • Other duties as assigned.

  • Must be able to move intermittently throughout the workday.
  • Must be able to cope with the mental an emotional stress of the position.
  • Must function independently and have flexibility, personal integrity and the ability to work effectively with the resident, personnel and support agencies.
  • Must be able to relate to and work with ill, disabled, elderly, emotionally upset and at times hostile people within the facility.
  • Must be able to lift, push, pull and move a minimum of fifty (50) pounds.
  • Must be able to assist in the evacuation of residents.
Other Requirements
  • May be required to attend activities outside the facility to stay abreast of new developments, best practices, and statutory and regulatory changes.
  • May be required to work holidays
  • Maintain the confidentiality of all resident care information.
