Responsibilities**GENERAL DESCRIPTION:**The social work assistant coordinates functions associated with patient information processing for the Child and Adolescent Partial Hospitalization Service (CAPHS). Assists and acts as a resource to clinical staff providing care to patients, ensure continuity of care, and support compliance with treatment. Monitors and ensures that all tasks assigned are completed in an accurate, efficient, and customer friendly manner. Performs functions associated with gathering and processing patient information for ambulatory care visits. Understand and have an awareness of potential presenting problems, mental diagnosis, and the unique needs of the patients and their families. This includes, but is not limited to the need for de-escalation, sensitivity, triage, and referral. Completes the responsibilities of reception, registration, e-Record task management, and program referral management via EMR referral work as well as other methods of incoming referrals.The social work assistant may participate and engage directly with patients in the therapeutic milieu, alongside licensed clinical staff, as needed. The social work assistant will collaborate with the primary therapists and community providers to facilitate timely access to services and support transition between levels of care; to provide access to concrete services that support treatment adherence.**RESPONSIBILITIES:**+ Assists in the performance of psychosocial evaluation of patients and their families. Assists the primary therapists and providers with agency contact both to collect data and to assess agency involvement.+ Functions as a member of the interdisciplinary treatment team for each patient assigned to his/her caseload. Consults and collaborates with team members and patient/family on the development of an appropriate discharge plan. May facilitate psycho-educational groups and provide coverage for other group leaders as needed.+ Coordinates the referral process to appropriate community resources to ensure that the discharge plan is implemented in the patient's discharge from the service. Provides social work follow-up to patients, families and agencies as needed after discharge to ensure continuity of care and integrity of the discharge plan+ Assists and supports patients and their families in their efforts to interact with the complex social service agencies involved in their care. Assists patient and families in interfacing with the hospital system and the finance office.+ Documents interventions in compliance with Division, Department and Hospital Policy, to include any safety plans or other documentation crucial to discharge planning and continuity of care.+ Attends Social Work Division and Department Staff meetings, as required. Participates in activities of the Social Work Division including, but not limited to timely completion of time reporting and other Hospital or Division forms; Hospital and Division mandatory in-service training programs and UHS reviews.+ Subscribes to the Departmental goal of creating a culturally competent environment by treating patient, families, trainees, and co-workers in a sensitive manner with appropriate attention to cultural differences.
