GRIPA is seeking a Social Work Care Manager to engage patients and collaborate with colleagues to deliver high quality, innovative, cost-efficient and supportive health care in our community.Job Objectives: The Social Work Care Manager will provide direct patient support both in-person and remote. They will be responsible for completing assessments, developing care plans, implementing interventions, and linking patients and families to community resources. The Social Work Care Manager will act as part of an interdisciplinary care team.Job Responsibilities:Work in settings across the care continuum including hospitals, provider practices and patient's home and/or community settingsComplete assessments that address social determinants of healthDevelop a care plan and implement interventionsProvide psychosocial supportProvide timely documentation of interventions with, or on behalf of patientsActively participate in on-line and in-person educational opportunitiesMonitor and measure patient progressRequired Experience:MSW or Masters in a related field or BSW with 5 (five) years of medical and/or community based Social Work/Care Management experienceKnowledge of local community resourcesExcellent communication and interpersonal skillsAbility to navigate complex medical and social systemsExceptional documentation and planning skills A collaborative and flexible spiritStrong computer skills (competence in Microsoft Office, particularly Word, basic Excel, Outlook, Teams, and familiarity with electronic medical records)Preferred Experience: Home based service delivery experienceBilingual skillsAdditional Requirements:This position requires local candidates only. It may be modified in the future and eligible for additional pay components.
