Job DescriptionUnder the supervision of Director of Social Work, but with reasonable independence and support HFM's Director of Care Management, the HFM Social Work Care Manager provides consultation and social work services to Highland Family Medicine patients and their families. Responsibilities include: Intake and clinical assessment, psychosocial assessments, referral to community resources, outreach, care coordination, brief behavioral health intervention as needed, psycho-educational support, and care management services to patients and families served by Highland Family Medicine. The SW Care Manager collaborates with HFM practice teams to assist patients who have identified social work needs. This role also provides consultation to the health care providers regarding patients' care needs and/or community resources.Salary Range:$29.70 - $36.19 an hourThe referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.Job Requirements
Education: MSW Required Experience (minimum required): Three years' experience in either community social work or hospital social work with a good appreciation for community resources preferredLicense/Certification Required: LMSW required after 18 months of hire Skills: Comfort with frequent use of computer systems as well as strong knowledge in Microsoft Excel, Word, PowerPoint, and Outlook. Strong interpersonal and organization skills, excellent written communication skills Ability to communicate effectively with people from diverse backgrounds, listen to patient's information and communicate this to the team. Ability to collaborate effectively in a team setting. Ability to maintain effective and professional relationships with patient and other members of the care team. Willingness to learn and carry out structured clinical protocols with patients in person or by telephone. Appreciates the need for confidentiality of patient information, accuracy of data collection and professional communication across all sectors of health care.
