CLINICAL SOCIAL WORKER / LICSW OR LCSW/ MGH Hybrid schedule (including onsite and remote/virtual schedule possible) The Integrated Care Management Program (iCMP) at MGH is looking for a clinical social worker with excellent care coordination experience and strong clinical skills. About iCMP: - The Integrated Care Management Program - iCMP - is a Primary Care practice-based care management/ coordination program. It is an innovative program that focuses on improving the health of high-risk medically, psychiatrically, and/or psychosocially complex patients. The objectives are to improve quality of care, patients and provider satisfaction, and reduce overall costs. This is accomplished through proactive patient outreach, engagement, and care coordination. - The iCMP Care Team members include Nurses, Pharmacists, Community Resource Specialists and Community Health Workers. About this Social Worker Job: - This position requires an enthusiasm for helping patients achieve their health care goals while working in close collaboration with a multidisciplinary team of providers. - This position is well suited for a social worker who understands the value of health equity in patient outcomes and who wants to contribute to the program's mission of supporting the care of clinically complex patients. Clinical Experience - Understanding of and comfort working with adult patients of all ages who have complex medical, behavioral health, and social determinates of health challenges - Ability to provide clinical psycho-social assessments and longitudinal treatment/clinical support of individuals, caregivers, couples and families - Strong understanding of psychiatric and family systems and ability to use this understanding to formulate succinct case summaries and care plans. - Demonstrated cultural competency and clinical skills for crisis intervention, safety concerns, SUDs, impacts of severe and persistent illness on individuals and family, aging and end-of-life planning Qualifications 1. Master's of Social Work (MSW) from an accredited program required. 2. Require either Massachusetts Licensed Independent Clinical Social Worker (LICSW) or Mass. Licensed Clinical Social Worker (LCSW) with relevant clinical experience working toward LICSW. 3. Bilingual Spanish candidates preferred. 4. Experience in the field of: Geri-psych, Substance Use Disorders and/or community mental health services preferred. Skills/Abilities/Competencies - Good organizational and time management skills. - Advanced crisis intervention/treatment/management skills. - Strong communication and collaboration skills - Strong interpersonal skills.
