The Opportunity We are seeking a full-time, 40-hour Clinical Social Worker to become an integral member of our ambulatory primary care team, working closely with the clinical and administrative staff to enhance the delivery of patient care services along the continuum of care. This is a collaborative approach to providing the best care! This employee will identify high-risk psychosocial factors of patients, help primary care staff understand the influence of those factors upon the course of medical care and assumes care coordination responsibility of the patients. Depending on each case there are times where the social work with provide treatment or intervention. In other cases, the social worker will be working with the treating outpatient clinicians (in or outside of our system) helping to ensure the treatment is focused, effective and carried through.The Key Functions of the Clinical Social Worker are:- Comprehensive assessment and case management- Patient navigation- Health Promotion- Crisis Management- Self-management education- Transitional care- Patient and family support- Liaison to community services- Short-term counseling- Advanced care planning- Team consultation Are you ready to bring your talent to this team and join us in moving health care forward?
