DescriptionJob Overview: The Social Work Support Specialist, as part of the clinical team, will collaborate and promote person-centered strategies and recommendations, engage in case reviews, and aggregate and organize data to assist in the care management process of the individuals that we serve.Location: Candidates should live in New York State, or in New Jersey/Connecticut close to the New York border. Essential Responsibilities:
- Serve as part of the Clinical Team for Person Centered Services.
- Provide case review or audit of individuals with mental health concerns.
- Serve as a consultant in situations where prior clinical expertise is warranted.
- Review and analyze data and trends.
- Design, implement, and present trainings as needed.
- Provide case review or audit of individuals with significant behavior issues and/or mental health concerns.
- Participate in mandatory staff meetings.
- Any other duties as deemed relevant by supervisor/organization
RequirementsSpecific Knowledge, Skills, and Abilities:
- Knowledge of developmental disabilities, mental health, and social determinants of health.
- Background in the school system
- Knowledge about IEPs, CSE, and DOE resources
- Knowledge of person-centered planning regulations.
- Demonstrates effective leadership skills.
- Demonstrates cultural competence.
- Demonstrates ethical and professional responsibilities and boundaries.
- Demonstrates capacity to use Health Information Technology to facilitate communication.
- Knowledge of confidentiality regulations/HIPPA.
- Ability to prioritize and complete work in a timely manner.
- Proactively approaches professional responsibilities.
- Demonstrate a comfort level of engaging with people with behavioral health needs.
Experience:
- Master's Degree in the field of psychology, social work, or a related field with at least 2 years of experience working with Intellectually and/or Developmentally Disabled individuals.
- Knowledge of OPWDD services and regulations
- Strong computer knowledge.
- Familiarity with person-first, person-centered planning concepts.
