Job Description:

Responsibilities:

The primary function of the Social Worker is to provide off-site phone support and guidance to medical and nursing staff. The social worker provides crisis intervention and supportive counseling to patients and families in all areas of the hospital. As a member of an interdisciplinary care team referral is made to appropriate community/social agencies. The social worker links patients and families to services in the hospital for support (i.e. Child Life and Pastoral Care). The social worker is to function in collaboration with psychiatry on site, Telemedicine or Licensed Clinical Social Worker to secure psychiatric placement for patients. It may be necessary for social work to report into the hospital depending on the nature of the case, but always for Level1 trauma and death and dying or deteriorating medical condition.
