It's Work That Matters.OverviewPurpose Statement/Position Summary: CHAT is accountable for implementing an institution-wide system for healthcare transition while patients are at CHLA and for transfer of care from CHLA to adult providers. This position will be within our Navigation Hub. The Navigation Hub is accountable for providing individualized case management support to referred patients and families. This case management is aimed at supporting the patient/family with transferring to adult providers, insurance navigation, conservatorship or healthcare power of attorney, and public benefits identification and applications. Under the supervision of a Clinical Social Worker, this position is responsible for providing comprehensive youth transitional case management services to at risk youth At-risk areas which may include the prevention and treatment of HIV, substance abuse or other At-risk condition. Will be responsible for assisting the medical care manager and patient care manager in care coordination, including follow up on referrals and data entry on behalf of team. This position will work closely with other Care Services team members to ensure that youth are enrolled in appropriate services. Responsible for the development of relationships with community-based agencies and providing technical assistance and training as requested. Participates in various community meetings. Minimum Qualifications/Work Experience: 2+ years' experience providing case management or clinical services to At-risk adolescents preferred. Experience in providing services to At-risk youth preferred. Experience in working with diverse populations preferred. Experience providing individualized case management support to individuals and their families. Healthcare and working with adolescent, young adults, and their families experience highly preferred. Experience working with insurance plans/supporting families to navigate insurance and working with other public benefit systems preferred. Education/Licensure/Certification: Bachelor's Degree required. BLS certification from the American Heart Association required. Pay Scale Information $38,272.00-$65,062.00 CHLA values the contribution each Team Member brings to our organization. Final determination of a successful candidate's starting pay will vary based on a number of factors, including, but not limited to education and experience within the job or the industry. The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this specific job description. Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.
