Job Summary and Qualifications1. Facilitation and coordination of care and services.
The Social Worker actively screens and identifies patients appropriate for social work intervention (e.g., including but not limited to requiring long term care placement, guardianship, substance abuse, new life altering diagnosis, complex family situations impacting decision making and discharge planning, debilitating chronic diseases, end of life issues, etc.).
May receive referrals from RN Case Managers and other sources for patients in need of social work intervention.
Actively participates in interdisciplinary rounds providing information on patients with psychosocial needs.
Communicates and collaborates with all members of the health care team regarding patients needs, plan and response.
Develops a plan for social work intervention and documents the plan, goals and response. Considers patient needs to include medical, physical, psychosocial, spiritual, financial and emotional aspects.
Completes psycho social assessments and high risk screens. Based on the assessment, develops and documents a short-term treatment plan.
Facilitates or participates in ad hoc patient/family conferences and with interdisciplinary team designed to gather information, problem identification, or to resolve issues around in-hospital and post-hospital care.
Educates health team colleagues about social services, including the role of the Clinical Social Worker. Participates in other educational programs as requested/appropriate.
Assists with explanation and obtaining Advanced Directives.
May collect, analyze, enter and report data related to patient care and participate in continuing quality improvement.
As appropriate, provides a brief (5-10 minute) admission assessment of each family or significant other to extend support and determine willingness, ability, and availability for caring for the hospitalized patient upon discharge. Documents findings.
Education Required: Masters Degree in Social Work. Licensed or license eligible as a Social Worker in the State of New Hampshire.
Experience: at least 1-3 years of experience as a Social Worker, with 1-2 years in the acute hospital setting preferable.
Knowledge of and demonstrated experience with a broad range of community services and resources.
