Provides direct social work assessments, problem identification, goal setting, and implementation of care plans. He/she collaborates with all disciplines to optimize patient outcomes; scope of services may include but is not limited to developing action plans to mitigate psycho-social barriers to discharge; patient/family engagement and education; employing interventions (crisis intervention, grief counseling, suspected abuse reporting) to address immediate needs.Carries out responsibilities in accordance with Baltimore Medical System Core Values, policies/procedures, and applicable civil laws. Is accountable for efficiently using resources and supporting the BMS mission.Education:* Bachelor's degree in Social Work* Master's degree in Social Work preferredCertification:* Licensed Social Worker (LBSW or LMSW)Experience:* Minimum of two years (of which two years is health care related field).* Must have expertise in one or more of the following areas:*
