Understanding of community health, welfare, and social agencies. Ability to communicate well and work effectively with patients and/or families, physicians, other home health staff, community agencies and public.Preferred/Optional: Home health experience.CERTIFICATIONS/LICENSES The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.Minimum Required: Current social work certification by applicable state. Applicable state board of licensing and regulation certification within 30 days from date of hire. BCLS within 30 days from day of hire. Must possess valid driver's license, automobile insurance and a reliable vehicle. Preferred/Optional: NoneJOB SUMMARYThe Social Worker- MSW provides Social Services to patients and their family. Must be capable of assuming some management responsibilities when department manager is unavailable. Must be able to work under varied and changing conditions on a daily basis.
