Veterans may stay for a short time or, in rare instances, for the rest of their life. It is a place where Veterans can receive nursing home level of care, which includes help with activities of daily living and skilled nursing and medical care. The mission of the CLC is to restore each Veteran to his or her highest level of well-being, to prevent declines in health and to provide comfort at the end of life. Veterans with chronic stable conditions including dementia are served in the CLC. The CLC Social Worker provides a broad range of clinical interventions for Veteran residents who are receiving services for long term care, rehab, respite, hospice and palliative care. Incumbent may provide cross coverage for GEC Social Work programs.Major Duties:* Possesses the knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.* Provides professional, independent social work services in the assigned area.* Assets and documents identified behaviors or symptoms of abuse, neglect, exploitation and/or intimate partner violence.* Implements different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychosocial, medical psychiatric, and medical problems to achieve treatment goals.* Participates in Interdisciplinary assessments and treatment planning meetings.* Serves as a advisor to the Resident Council and act as intermediaries between the Resident Council and CLC Administration as needed.* Provides interventions with Veterans and their families/caregivers who are experiencing a wide range of complicated medical, behavioral health, financial, legal, and psychosocial problems.* Facilitates discharge planning in conjunction with other disciplines to ensure safe disposition planning to home, nursing homes, residential communities, or other settings.Work Schedule: M-F, 8am-4:30pmRecruitment Incentive (Sign-on Bonus): AuthorizedEDRP Authorized: Former EDRP participants ineligible to apply for incentive.
