We are seeking a Social Worker - Full-Time.
- $60K to $70K annually based on skills and experience.
Social Worker Qualifications:
- Must have a Bachelor's Degree in Social Work or related field, Masters Degree preferred.
- One year in Long-Term Care as a Social Worker required.
- Proficiency using PCC (Point-Click-Care) is required.
Social Worker Introduction: Under the supervision of the Director of Social Work, assist the residents to attain or maintain the highest practicable physical, mental and psychosocial well-being, and in navigating the health care system.Social Worker Job Summary:
- Interview residents and their families.
- Provide assessments and counseling to residents and families.
- Develop care plans based on the individual need of residents.
- Provide crisis intervention as needed.
- Provide grief and supportive counseling.
- Coordination of discharge planning and case management.
