Examples of Duties
- Meet the needs of the individual as directed by the Court, including:
- Assist individual to locate, secure and move into housing
- Provide consent to medical and psychiatric care
- Identify service needs and coordinate service delivery with County departments and private providers
- Seek benefits and services and complete associated applications and documents
- Identify assets and liabilities and monitor financial disbursements made on individual's behalf
- Prepare reports to the Court
Actively participate in the development and/or planning of individual program activities.- Establish and maintain a relationship with the individual through regular contact which includes at least one quarterly face to face contact
- Provide input to the team in the planning/development of services through attendance at meetings with providers
- Establish collaborative working relationships with county and private service providers, community providers and family/significant others
Respond to crisis and emergencies in a timely and effective manner.- Assess the apparent crisis and determine the safest and most practical method of intervention
- Consult with service professionals and other resources for support
- Coordinate after-hour crisis plans with Rice County Social Services Crisis Line personnel
Attends required training sessions and meetings as directed.Performs other duties as required. QualificationsMINIMUM QUALIFICATIONS: Bachelor's degree in Social Work or a related field and two years of experience working with vulnerable individuals. Valid driver's license required. PREFERRED QUALIFICATIONS:Experience working within the court system preferred. Master's degree in Social Work and previous experience as a county Adult protection Worker preferred.Supplemental Information HIRING RANGE / UNION: $34.79 - $36.37 per hour / SS Unit - Grade 12HOURS: Part-time (25 hours per week). Typical work hours between approximately 8:00 a.m. - 4:30 p.m. Monday - Friday. May occasionally work limited hours on weekend, holiday, or evening.
