Community Healthcare Network is seeking a Full-Time Social Worker/ Healthy Steps Program who will work along with and in support of pediatricians and in partnership with the clinical and the administrative team. The Social Worker provides support for families with children age 0-5 years and provides caregiver support to facilitate the development of attachment, self-regulation skills, and family resiliency through one-on-one and group interactions. JOB FUNCTION: Responsibilities include, but are not limited to: Conduct in-office or remote visits with parents, caregivers and their young children, either jointly with the physician/ nurse practitioner or alone. Work closely with providers around care coordination, goal setting, counseling, coaching and education about key aspects of a child's development. Closely track progress made in ongoing checkups that look at both child development and family factors.
