Work Schedule: Full-time, 100% FTE day shift. Monday - Friday between the hours of 8:00am - 4:30pm or 8:30am - 5:00pm, occasional evening coverage as needed, no weekends required. Hours may vary based on the operational needs of the department.

Social Worker - HIV Program - Bilingual to:
- Assess psychosocial needs and apply brief interventions in order to support the medical treatment plan.
- Utilize interventions to enhance patient and family functioning and emotional well-being and access appropriate and agreed upon services.
- Collaborate and coordinate care to enhance the patient experience and the achievement of collaborative goals.
- Collaboratively develop patient goals, monitor progress, address barriers to goal achievement, and evaluate outcomes of individualized care plans.
- Provide training and guidance and mentor staff and physicians on the management of complex psychosocial patient issues in line with the organizational academic mission.

Education:Minimum - Master's Degree in Social Work
Work Experience:Preferred -
- Two (2) years prior hospital based social work experience
- Two (2) years of prior hospital or community-based social work if supporting the HIV Clinic.
Certifications:Minimum - Licensed in the state of Wisconsin with one of the following within 90 days of hire:
- Certified Advanced Practice Social Worker (CAPSW)
- Certified Independent Social Worker (CISW)
- Licensed Clinical Social Worker (LCSW)
- Bilingual in Spanish
