**A Brief Job Description:**
**SUMMARY**
This position is responsible for the delivery of Social Work services to identified home care clients. The Social Worker provides support and counseling to referred clients and facilitates financial and long-term care planning, as needed. The Social Worker is responsible for communicating with health care team and documenting all care activities.
**STATUS:** Part Time 32 Hours Per Week
**LOCATION:** Dundee Office- Covering Yates, Schuyler and parts of Livingston and Seneca counties (as needed)
**DEPARTMENT:** Home Care
**SCHEDULE:** Monday-Friday/Day Shift
**ATTRIBUTES**
* Bachelors Degree in Social Work with a minimum of two years direct Social Work experience; Masters Degree in Social Work preferred
* Home visiting experience strongly preferred
* Possession of a valid NYS drivers license
**RESPONSIBILITIES**
* **Patient/Family Services.**Interprets to the home health team the social factors in the patient situation that may have bearing on his/her progress. Helps patient and family understand and accept the illness and to anticipate its accompanying adjustments.
* **Resource Management.**Coordinates resources to alleviate the financial impact of illness and assists patient and family in planning for payment of medical care. Determines the need for continued social work service and draws upon other community agencies that can further assist the patient and family.
* **Discharges.** Assist with discharge planning from Lifetime Care program(s).
**EDUCATION** BS: Social Work (Required)
**PHYSICAL REQUIREMENTS** S Sedentary Work Exerting up to 10 pounds of force occasionally Sedentary work involves sitting most of the time, but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally and all other sedentary criteria are met.
