*Includes an optional $4,000 sign-on bonus with 1 year contract post orientation!**Cookeville Regional Medical Center is seeking a Full-Time Social Worker to join our Home Health team!*The *Home Health Social Worker* is responsible for providing a wide range of social services in accordance with the hospital mission and objectives. Services provided by Social Service include assisting patients and their families in addressing social, emotional, and economic issues associated with illness or injury. Crisis intervention, discharge planning, continuity of care, financial assistance and referrals to community resources, while interacting with interdisciplinary team are some of the assigned duties.
