The Clinical Social Worker utilizes clinical expertise to perform psychosocial assessments, develop and implement plans of care appropriate care team, assess crisis situations to appropriately provide needed resources and or referrals for the patient, and to provide family and individual treatment as indicated. This position works collaboratively with patients, patient's significant other, families, healthcare providers, insurers, community resources, and all other involved parties.

As a Home Hospice Social Worker you need to know how to:

  • Assess for appropriateness of level of care setting from admission through discharge.
  • Identify patients requiring case management and take the lead as care manager for those requiring intervention, especially with socially complex cases.
  • Collaborate with the health care team to develop a plan of care by participating in patient care rounds.
  • Assure the plan of care is individualized for patients with a chronic disease to support patient/family self-management.
  • Serve as a support to nursing partners in the area of social services, Medicaid applications, disability eligibility, ETOH resources and homeless resources.

Minimum Qualifications

  • MSW, Graduate of an accredited school of social work, sociology, or related field, required.
  • Current license in good standing as a Medical Social Worker (LSW or LCSW) in the state of practice, required.
  • Ability to maintain reliable transportation, maintain a current drivers license in good standing and current auto insurance, required.
  • Current Basic Life Support for Healthcare Providers (BLS-HCP) course completion card from the American Heart Association, required.

Preferred Qualifications

  • Certified Social Worker in Health Care (C-SWHC), preferred.
  • Hospice and Palliative Care Division: - Advanced Certified Hospice
  • Palliative Social Worker (ACHP-SW), preferred.
  • Advanced Palliative Hospice Social Worker Certification (APHSW-C), preferred.

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations

.- and -

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc

.- and -

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.

- and -

Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc)

.- and -May be expected to stand in a stationary position for an extended period of time.

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.

$28.15 - $43.42
