Together, we improve the health of patients and their families, making our communities - and the world - strongerThe social workers on the medicine service line, care for our most vulnerable, at risk patients in the hospital. Homeless, behavioral health patients who require security watch, drug and alcohol addicted, victims of sexual and domestic violence, the elderly who are victims of physical and financial abuse, those who suffer from mental health issues, ethics cases, guardianship, the undocumented and those who find themselves in the unfortunate situation of having no one left to care for them are screened, assessed and appropriate planning put into place by the social worker. The social work also refers all patients to skilled facilities, sub-acute rehabs, LTACH, hemodialysis centers, assist with transplant assessments, adult protective services, acute rehabs, psychiatric placements and hospice care. Working closely with families who are struggling with end of life decisions for their loved one and provide emotional support and comfort to the patient and their family members. The social worker also attends court for guardianship cases, leads and coordinate the goals of care, interdisciplinary teams and legal and ethics.This is a day shift, Monday - Friday position with rotating weekend/holiday coverage.Preferred Criteria
  • At least one year of inpatient Medical/Surgical experience
  • Discharge Planning experience
Required Criteria
  • Master's Degree in Social Work (MSW)
  • New York State License as a Licensed Masters Social Worker (LMSW)
