St. John Community allows older adults to move through a continuum of care from residential living to skilled nursing, depending on the need. Serves as an advocate to the Resident in a person center directed culture. The social worker is a part of the interdisciplinary team, responsible for identifying and developing care plans that address the psycho-social needs of the resident/patient. For assigned residents/patients, insure appropriate care plans, act as resident/patient advocate, staff education as appropriate. Develop goals for residents/patients and work with residents/patients to achieve these goals. Acts as liaison and provides educational support to residents/patients and their families.
