Cedar Manor Nursing & Rehabilitation Center is searching for Full Time Social Worker to become a part of our facilitys compassionate care-giving team. The Full Time Social Worker must have experience in long term care.

Job Summary For Social Worker :

• Identify the medically-related social service needs of residents and assure needs are met. This includes maintaining or improving their ability to mange their everyday physical, mental and psychosocial needs.
• Assist with discharge planning.
• Conduct Social Services assessments per Department of Health regulations.
• Participate in Inderdisciplinary Care Plan Meetings, Discharge Meetings, Case Mix Meetings and other meetings as assigned.
• Complete MDS accurately and timely.
• Complete admissions in a accurately and timely manner.
• Assist with Resident Council as needed.
