Job Requirements: - Provides direct social work services to patients in their homes
- Works with the interdisciplinary team in integrating social work services into the plan of care
Education - Master's Degree in Social Work from an accredited School of Social Work
Required Licensure/Certification - Must have Maryland License in Social Work (LCSW-C) with counseling experience
- Must obtain current CPR certification
- Must have valid Maryland Driver's License
Work Experience - Home Care experience is preferred
- Experience with management of adult population
- Clinical experience in counseling and casework for the terminally, chronically, and acutely ill adult population
