Services are provided in accordance with professional standards and in consultation and/or collaboration with the multi-disciplinary health care team, as warranted, to assist and encourage patients, families, and significant others to utilize medical care constructively to assure the achievement of an optimal level of functioning in all patient care settings in the community, as well as alternate levels of care.*Essential Job Responsibilities:** *Counseling and Case Management:* Responsible for own decisions and expertise in the counseling process. Ensure proper linkages for support and identified needs for ongoing patient care.* *Team Collaboration:* Work effectively in a team environment with other caregivers. Meet or exceed productivity standards as set by the department.* *Psychosocial Assessment:* Complete assessments of psychosocial needs, considering age, cultural concerns, spiritual beliefs, and special needs, to determine appropriate services and treatment plans.* *Ongoing Counseling:* Provide ongoing professional counseling (individual, family, and group) and case management services as indicated, facilitated by ongoing reassessment to determine evolving needs in the hospital and community.* *Care Planning:* Participate in multi-disciplinary care planning, including interpretation of psychosocial needs assessments/reassessments. Complete lethality assessments and screenings for psychiatric/psychological consults as indicated.* *Education:* Provide ongoing education to patients, families, and significant others related to psychosocial, medical, and associated needs, in support of and in collaboration with the multi-disciplinary team.* *Documentation:* Document case activity per department documentation policy, including assessment, intervention, reassessment, discharge and aftercare planning, patient/family education, linkage and referral, and all other relevant case activity.* *Resource Evaluation:* Evaluate UBMD Pediatrics and community resources and facilitate utilization of these resources by linking patients, families, and significant others to public and private agencies to meet identified needs for ongoing care, resources, and support in the community.* *Advocacy:* Advocate for patients, families, and significant others, including assistance in the development of self-advocacy skills and maximization of a patient's level of independence and improvement of their overall quality of life.* *Specialty Programs:* Demonstrate understanding and knowledge of specialty disease programs and associated special needs of patients, families, and significant others, and adhere to program policies and mandates.* *Community Agencies:* Assume responsibility for familiarity with nursing homes, sub-acute units, rehabilitation centers, domiciliary facilities, and other home care and community agencies.* *Financial Assistance:* Provide information and referral to assist patients, families, and significant others with financial issues associated with hospitalization, illness, ongoing medical care, and continued care/services in the community.* *Support to Staff:* Provide ongoing formal and informal support to administrative, medical, and other staff. Develop and maintain positive working relationships with staff throughout UBMD Pediatrics, as well as with public and private community agency personnel.* *Committee Participation:* Participate in committees and assist on projects involving UBMD Pediatrics and in the community as indicated. Participate in continuing professional education: attend seminars, in-services, and conferences; and comply with department requirements for supervision.* *Professional Development:* Maintain active membership in professional organizations, and develop and deliver in-service programs for department staff, professional organizations, and community groups as indicated.* *Meeting Attendance:* Assume responsibility for staff meeting attendance and for content of posted and circulated information.* *Statistics and Reports:* Maintain and assure timely submission of accurate statistics and reports.* *Compliance:* Comply with Federal, State, and any regulatory agency requirements, UBMD Pediatrics departmental policies, professional standards, and procedures including those that apply to professional conduct, documentation, standardized abbreviations, confidentiality, case activity, infection control, and safety.* *Coverage and On-Call:* Provide coverage for other social work staff as directed.. Participate in the orientation of new hires. Assume responsibility for social work students. Demonstrate awareness and understanding of cultural diversity in daily activities.* *Continuous Quality Improvement (CQI):* Participate in the CQI process and employ CQI principles in daily activities.* *Professional Literature:* Assure familiarity with current professional literature related to Medical Social Work, Case Management, and related specialty programs.* *Other Duties:* Perform other related duties as assigned by authorized personnel or as required to meet emergencies.* *Liaison and Education:* Act as a liaison between families and schools. Help families develop 504 plans. Participate in the education of school nurses.*Education*Requires a Master's Degree in Social Work.*Experience*A minimum of two years' experience in a related field and healthcare setting is a plus.*Knowledge, Skills & Abilities** *Healthcare Knowledge:* Knowledge of the healthcare field; medical practice clinical and administrative systems, departments, and practices, including clinic financial policies and reimbursement payment requirements.* *Counseling and Conflict Resolution:* Knowledge of counseling, conflict resolution, and customer service principles and applications.* *Research Methods:* Knowledge of research methods to identify issues and clarify policies. Understanding of medical terminology.
