  • JOB SUMMARY:
  • The Social Worker will intervene in the social environment of the Resident, including interactions with other Residents, family, staff and the community in order to promote the optimal physical, psychological and social functioning of the Resident.
  • ESSENTIAL DUTIES:
    • Documents and maintains social work progress notes from admission through discharge.
    • Completes assessments to address the psychosocial needs of the resident.
    • Develops and implements a comprehensive care plan with the multi-disciplinary team, for the Resident.
    • Assures that Residents right to fair and equitable treatment, self-determination individuality, privacy, property and civil rights, including the right to file complaints, are followed.
    • Efficiently identifies and responds to Resident needs.
    • Completes MDS in a timely manner.
    • Coordinates and facilitates resident care plan meetings.
    • Coordinates with inter-disciplinary team and facilitates discharges to home, including referrals for services.
    • Reviews and maintains Advance Directives.
    • Coordinates referrals for in-house Psychology and Psychiatry
    • Provides timely reports and demonstrates accountability for work functions.
    • Renders safe and efficient care based on Weinberg Campus Policies and Procedures, as well as NYS Department of Health Guidelines.
    • Visually assess resident and render appropriate care.
    • Other duties as assigned by the Director of Social Work.

      • Ability to work with people from various socio-cultural backgrounds.
      • Ability to accept and utilize supervision.
      • Ability to work independently.
      • Ability to maintain complete and full confidentiality of Resident Records.
      • Able to respect the privacy, dignity, and confidentiality of all residents.
      • Must be able to communicate with resident, supervisors, and coworkers effectively.
  • WORKING CONDITIONS:
      • Working with the elderly in a residential and clinical environment.
      • Risk of exposure to human bodily fluids.
  • PERSONAL OR PHYSICAL REQUIREMENTS:
  • The physical demands described here are representative of those that must be met by an Associate to successfully perform the essential functions of this job. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.
      • Be able to maneuver throughout the Campus as well as to and from other campus sites.
      • Able to push, pull, lift, and carry items up to 30 lbs.
      • Able to stoop, twist, bend, squat, kneel, and reach items above, at, or below shoulder level.
      • Able to push residents in wheelchairs.
  • EDUCATION:
      • Bachelor's Degree in Social Work or Related Field (required).
  • EXPERIENCE:
    • One (1) year of experience in Social Work, Human Services, or Related Field (preferred).
    • LTC Experience (preferred).
